ABSTRACT

【Objective】 To analyze the cause of single-ELISA reactive of four blood screening items in 18 blood stations in Henan, so as to provide the basis for improving the quality of blood screening. 【Methods】 The single-ELISA reactive rate of HBsAg, anti-HCV, HIV Ag/Ab and anti-TP of 18 blood station laboratories in Henan throughout 2019 was calculated, and the causes were analyzed according to different ELISA reagent combinations and gray area settings in each laboratory. 【Results】 The overall single-ELISA reactive rates of HBsAg, anti-HCV, HIV Ag/Ab and anti-TP were 1.740(2 154/1 237 789), 0.564‰(698/1 237 789), 1.421‰(1 759/1 237 789) and 1.561‰(1 932/1 237 789), respectively, showing significant differences by detection items (P <0.05). Person correlation analysis showed that the single-ELISA reactive rate was independent of the gray area settings.but dependent on laboratories and reagent combinations. The single-ELISA reactive rate of HBsAg, anti-HCV, HIV Ag/Ab and anti-TP in D laboratory was the highest and higher than that in other labs using the same reagent.The laboratories with high HBsAg single-ELISA reactive rate were mostly those using a combination of imported reagents and domestic reagents, including the top 6 laboratories. The laboratories with high anti-HCV single-ELISA reactive rate were mostly those using certain domestic reagents. No obvious rules was noticed by single-ELISA reactive for anti-HIV. Laboratories with high anti-TP single-ELISA reactive rate were mostly those using combination 4. 【Conclusion】 The HBsAg single-ELISA reactive rate was the highest in the four blood screening items of blood station laboratories in Henan. The single-ELISA reactive rate is related to the laboratory itself and the reagent manufacturer, suggesting that laboratory quality control should be strengthened and proper reagent combination should be selected to reduce the waste of blood.

ABSTRACT

【Objective】 To analyze the HIV-, HCV- and HBV- NAT yield rate in different areas of Henan province, so as to provide the basis for disease prevention and control as well as the establishment of a unified quality control standard for nucleic acid testing(NAT) in the Henan province. 【Methods】 The number and prevalence of NAT yielding samples with isolated infectious virus, namely HIV, HCV and HBV, in 18 blood stations in Henan province from 2017~2019, as well as the trends were analyzed. The NAT quality of each laboratory and each testing system was analyzed according to the ratio of reactive individual donation(ID) results to reactive minipools(MP). 【Results】 The HBV, HCV and HIV ID-NAT yield numbers in 3 501 251 blood donations were HBV 2 606(74/100 000), HCV 21 (0.63/100 000), and HIV 34(1.00/100 000). The HBV ID-NAT yield rate showed an upward trend in the whole province from 2017 to 2019, while the prevalence of HIV and HCV ID-NAT yield didn′t differ significantly during three years. 5 kinds of NAT detection systems were applied in 18 blood centers. among which Ⅰ, Ⅱ, Ⅳ and Ⅴ were triplex detection systems. 2661 ID-reactive samples were implicated in 5 595 MP-reactive samples, with a resolution rate of 47.56%. The resolution rate of triplex NAT system Ⅰ, Ⅱ, Ⅳ and Ⅳ was 39.63%~47.95%, 40.43%~54.36%, 51.61% and 70.00%~45.45%, respectively. An upward trend in triplex NAT resolution rate was observed in 8 laboratories, i. e.B, D, E, F, I, K, L and Q, and an descending trend in A and C. The NAT system Ⅲ, a ID-NAT system, was used only by laboratory C, presenting a NAT-yield rate of 0.19% (282/145 474) and resolution rate of 46.45% (131/282). 【Conclusion】 The majority of NAT-yield of one infectious virus in Henan province is HBV, presenting annual increasing trend. The quality management of NAT laboratories should be strengthened as the divergence was seen in the performance of different NAT laboratories.

ABSTRACT

Objective@#To investigate the relationship between the expression of leptin protein and clinicopathological features and prognosis of gastric cancer.@*Methods@#The cancer tissue specimens of 112 patients diagnosed with gastric cancer in Shaanxi Provincial Cancer Hospital from January 2009 to December 2012 were collected. Random number table method was used to select 50 cases of paracancerous normal mucosa tissues as the controls. The expression of leptin protein was detected by using immunohistochemistry. And the relationship between the expression of lepin protein and the clinicopathological features and prognosis was analyzed.@*Results@#The positive rate of leptin protein in gastric cancer tissues was higher than that in paracancerous normal gastric mucosa tissues, and the difference was statistically significant [68.8% (77/112) vs. 34.0% (17/50), χ2 = 17.139, P < 0.01]. The expression of leptin protein was associated with tumor vessel invasion, invasion depth and TNM stage (all P < 0.05). The expression of leptin protein was positively correlated with tumor invasion depth and TNM stage (r = 0.265, P = 0.005; r = 0.318, P = 0.001). The 5-year total survival rates of patients with positive and negative expression of leptin protein were 65.2% and 78.4%, respectively, and the difference was statistically significant (χ 2 = 9.526, P = 0.002). The 5-year total survival rates of patients who received neoadjuvant chemotherapy or not were 71.7% and 67.6%, respectively, and the difference was not statistically significant (χ 2 = 2.321, P = 0.128). In patients with positive expression of leptin protein, 5-year total survival rates of patients who received neoadjuvant chemotherapy or not were 47.4% and 74.2%, respectively (χ 2 = 6.336, P = 0.012). In patients with negative expression of leptin protein, 5-year total survival rates of patients who received neoadjuvant chemotherapy or not were 92.3% and 18.2%, respectively (χ 2 = 14.237, P < 0.01). Cox multivariate regression analysis showed that leptin protein expression, lymph node metastasis, pathological grading, vessel invasion and TNM stage were independent prognostic factors for patients with gastric cancer (all P < 0.05).@*Conclusion@#Leptin protein is highly expressed in gastric cancer tissues, and it can be used as a marker for judging the prognosis and chemotherapy effect of gastric cancer patients.

ABSTRACT

Objective To investigate the relationship between vascular endothelial growth factor C (VEGF-C) and lymphatic vessel density (LVD) with clinicopathological features of the patients in stage Ⅱ colorectal cancer.Methods Fifty-seven tissues in stage Ⅱ colorectal cancer and 57 normal mucosal tissues in Shaanxi Provincial Cancer Hospital from January 2005 to December 2010 were collected.The expression of VEGF-C and LVD were detected by using immunohistochemical method.The relationship between VEGF-C and LVD with the clinicopathological features of colorectal cancer was analyzed.Results The expression of VEGF-C and LVD in cancer tissues was higher than that in normal tissues (6.80±1.24 vs.2.07±0.17,t =3.773,P =0.000;3.19±0.53 vs.1.84±0.26,t =2.276,P =0.025).There were statistical differences between VEGF-C and LVD in different tumor diameter (4.00±0.54 vs.11.26±2.89,t =-3.049,P =0.004;1.89±0.37 vs.5.27±1.12,t =-3.376,P =0.001) and relapse or metastasis (4.74±1.31 vs.10.62 ±2.39,t =-2.158,P =0.039;1.86±0.45 vs.5.65±1.06,t =-3.778,P =0.000).LVD in VEGF-C positive group was higher than that in VEGF-C negative group (4.16±0.64 vs.0.21±0.11,t =3.503,P =0.001).The expression of VEGF-C was positively correlated with LVD (r =0.892,P =0.000).Conclusions The expression of VEGF-C in stage Ⅱ colorectal cancer is correlated with LVD,as well as tumor size and relapse or metastasis.VEGF-C and LVD could also play important roles in the occurrence,development and metastasis in stage Ⅱ colorectal cancer.

ABSTRACT

Objective To study the relationship between expression of leptin and clinical pathogenesis and prognosis of stage Ⅱ colorectal cancer.Methods The clinical data of 102 patients with colorectal cancer admitted to Shaanxi Provincial Tumor Hospital between January 2005 and December 2010 was collected and statistically analyzed.The expression of leptin in postoperative colorectal cancer tissues and 40 normal colorectal tissues was detected by P-V immunohistochemical technique.Results The expression of leptin in cancer tissues were obviously higher than that in normal tissues [59.8 % (61/102) vs.17.5 % (7/40),x 2 =20.605,P =0.000].There was no association between leptin expression level and gender,age,tumor location,pathological grading,pathological types,tumor size,invasion depth and body mass index (all P < 0.05).Cox multivariate regression analysis showed that leptin expression (P =0.041) and adjuvant chemotherapy (P =0.008) were independent factors affecting OS in patients with stage Ⅱ colorectal cancer.The patients with positive expression of leptin could not benefit from the adjuvant chemotherapy (P =0.259),and patients without leptin expression had improved survival when treated by adjuvant chemotherapy (P =0.021).Conclusion The expression of leptin in stage Ⅱ colorectal cancer tissues is obviously higher than that in normal tissues.Leptin is associated with chemotherapy resistance and therapy-independent prognosis.

ABSTRACT

Objective The aim of the study was to evaluate the hearing performance of a new transcutaneous bone conduction hearing aid.Methods One hundred and nine patients with conductive or mix hearing loss and eleven patients with single sided sensorineural hearing loss from four tertiary referral centers were tested under unaided and aided conditions with the sound processors on testbands.Free field test was performed to obtain the hearing thresholds and speech recognition thresholds in quiet.Adverse events were also documented by a self-reported questionnaire.Results ①Conductive or mixed hearing loss patients:for aged≥6 yrs subgroup,the bone conduction PTA average of aided sides was 18.55±8.99 dB HL.For aged<6 yrs subgroup, the bone conduction ABR hearing threshold average was 18.33±8.36 dB HL.②Statistically significant improvements in free field hearing thresholds were seen when compared aided hearing with unaided outcomes.The average PTA (pure tone average in free field) was measured at 32.21±10.00, 37.33±14.15, and 34.38±10.76 dB HL respectively in conductive or mixed hearing loss aged≥6 yrs subgroup;aged<6 yrs subgroup and SSD subgroup.③In aided situations;speech recognition thresholds in quiet were significantly better compared to unaided situation in patients aged≥6yrs.④None of patients presented adverse events related to the devices.Conclusion The Sophono bone conduction hearing aid has satisfactory hearing performance and can be considered an alternative device for patients with conductive or mix hearing loss or with SSD.

ABSTRACT

Objective:To research the clinical application of tympanic cavity ventilation tube placement in canal wall down tympanomastoidectomy. Method:There were 52 patients with otitis media received canal wall down tympanomastoidectomy. We placed ventilation tube in posterior hypotympanum during the surgery. After surgery, all patients were followed up in 12 days, 1 month, 2 months, 6 months, and 1 year. The ventilation tubes were withdrew 6 months later, and then the hearing level and tympanic cavity status was evaluated after another half year. Result:All cases were followed up at least 1 year after surgery. Forty-four patients' tubes were removed 6 months later, but other 8 tubes had been extruded within 1 to 3 months. After 1 year, all 52 cases of ears were dry; 39 patients' tympanic membranes were stable, the postoperative complications included tympanic membrane retraction in 7 cases, middle ear effusion in 4 cases, and tympanic membrane perforation at anterior-inferior quadrant in 2 cases; CT examination indicated that 39 patients' aeration between tympanic cavity and Eustachian tube bony portion was normal, 9 patients' tympanic cavity aeration was smaller, and 4 patients' cavity was obstructed; 52 patients' average air conduction thresholds were 12.5 dB lower than that of preoperation. Conclusion:One-stage tympanic cavity ventilation tube placement is a simple, safe and effective method in canal wall down tympanomastoidectomy for middle ear diseases with Eustachian tube severe stenosis or obstruction in bony portion.

ABSTRACT

OBJECTIVE@#Dissecting adult cadaver's tragal cartilage and researching its clinical application in ear surgery.@*METHOD@#Dissect the bilateral tragal cartilage of 22 adult cadavers immersed in formalin (both of male and female are 11). Measure and compute the average value of the length, width, thickness and area. Summarize autologous tragal cartilage's clinical application in ear surgery.@*RESULT@#The statistic values of male tragal cartilage were: length (22.55 ± 0.89) mm, width (19.00 ± 1.09) mm, thickness (1.04 ± 0.09) mm, and area (315.70 ± 32.57) mm2. The statistic values of female respectively were (19.36 ± 0.86) mm, (15.73 ± 0.69) mm, (0.93 ± 0.06) mm, and (229.64 ± 13.97) mm2. Tragal cartilages were utilized in 419 middle ear surgeries in my department, including tympanoplasty(type I 189 cases, type II and III 116 cases), atticotomy (65 cases), and the repair of the lateral skull base (3 cases). The postoperative effect was satisfactory.@*CONCLUSION@#Tragal cartilage is in the operation region, which is convenient to be harvested and shaped. Hence, the donor can satisfy the requirement of general ear surgery and it is suitable for widely application in ear surgery.

ABSTRACT

OBJECTIVE@#To investigate the reconstruction method of lateral attic wall with tragal cartilage and temporalis fascia graft. And analyze the postoperative result of its clinical application.@*METHOD@#From Jan 2005 to Jul 2014, 45 patients whose middle ear disease were limited to attic received this surgery in our department. Among 31 cases of cholesteatoma otitis media and 14 cases of external auditory canal cholesteatoma were included. In order to expose the attic fully, we operated epitympanotomy through retroauricular incision and then removed the scutum and lateral attic bone wall. After eliminating the lesions, we reconstructed the lateral attic bone wall with tragal cartilage, covered the cartilage with temporalis fascia and then repaired the tympanic membrane and external ear canal skin. After surgery, all patients were followed up at 10 days, 1 month, 2 months, 6 months and 1 year.@*RESULT@#Two months after surgery, 45 patients' achieved one-stage wound healing. Six months later, all of the patients' operation area had epithelized completely. After 1 year, 37 patients had recovered the normal shapes and stable audition; 7 cases patients have different level tympanic membrane retraction; 1 patient suffered from tympanic membrane retraction and recurrent cholesteatoma.@*CONCLUSION@#With regard to the lesion limited to the attic, we can remove it by operating epitympanotomy through retroauricular incision, and then reconstruct the lateral attic wall with tragal cartilage and temporalis fascia. By the support of the cartilage, we can keep the epitympanic aeration, reduce the retraction of pars flaccida membrana tympani, and maintain the fundamental shape of lateral attic wall.

ABSTRACT

Objective To investigate the changes of pulsatile tinnitus on HRCT and MRI CE 3D-SPGR image.Methods CT and MRI images of 1 5 pulsatile tinnitus patients were retrospective analysis.All 1 5 patients underwent temporal bone HRCT and MRI CE 3D-SPGR scan.Abnormal changes on CT and MRI image,which caused the pulsatile tinnitus,were compared.Results In 1 5 patients,9 sides (8 cases)showed abnormal sigmoid sinuses including 4 sides of sigmoid sinuses diverticulum,5 sides sigmoid sinu-ses uncovering,8 sides dominant sigmoid sinuses.The high jugular bulb was showed in 10 sides (8 cases),the thick emissaria mas-toidea in 5 sides (4 cases),bilateral semicircular canal dehiscence in one case,glomus tympanicum tumor in one case and the cochle-ar nerve was constricted by small vessel in one case.Conclusion Cranial base vessel abnormity can be visualized more precisely on MRI CE 3D-SPGR image than that on HRCT.On the contrary,the detection of temporal bone abnormalities is superior on HRCT.

ABSTRACT

OBJECTIVE@#This paper highlights the therapeutic effect of tragus cartilage and temporalis fascia graft on repairing bone and skin defect in external auditory canal.@*METHOD@#Forty-one surgical patients were recuited, including 39 cases of external auditory canal cholesteatoma and 2 cases of osteoma. External auditory canal bone wall and skin defect and mastoid air cells exposure were found during operation. We followed up these cases periodically (in 3 weeks, 3 months, 6 months and 1 year later respectively) after repairing the patients' external auditory canal bone wall and skin with tragus cartilage and temporalis fascia graft.@*RESULT@#Twenty-nine cases healed perfectly while other 12 cases grew small granulation in 3 weeks. After shaving granulations and tamping Tela Iodoformum, wounds healed in the following week. The epithelization of the external auditory canal in our patients was complete after 3 months and no stenosis were found. Natural morphology of external auditory canal was maintained without mastoid air cells exposure after 6 months to 1 year of time.@*CONCLUSION@#It is not only convenient to acquire the tragus cartilage and temporalis fascia, but also easy to repair the defect of external auditory canal bone wall and skin. It can reconstruct the wall of external auditory canal, obliterate mastoid cells and perfectly maintain the normal form of external auditory canal via the cartilage.

OBJECTIVE@#To observe the short-term efficacy of autogenous bone pate and Palva graft for obliterating huge remnant mastoid cavity in canal wall down approach.@*METHOD@#Retrospective analysis clinical data of twenty-one cholesteatomatous cases operated by one surgeon from 2004 to 2007. In twelve cases, simultaneous III type tympanoplasty (Sheehy, P. O. P) was performed. Other 9 cases had undergone mastoidectomy elsewhere before the admission. Six of them were still draining with huge remnant mastoid cavity, and the rest three patients had relapsed cholesteatomas with intermittent draining and huge mastoid cavity. Normal saline solution perfusion was used to measure the volume of remnant mastoid cavity. The criterion of huge remnant mastoid cavity is more than 8 ml.@*RESULT@#Of twelve primary cases with III type tympanoplasty, 11 patients maintained a small, dry, and healthy mastoid cavity after twenty-seven days. The average increase of hearing level of them was 17.5dB, and the air-bone gap is less than 20 dB. Of one patient, bone pate was infected and was discharged. A dry mastoid cavity was achieved until fifty-five days after surgery. The patient is keeping a big air-bone gap caused by displacement of ossicle chain prosthesis. Just eighteen days later, other nine cases of revision mastoidectomy achieved a small, dry, and healthy mastoid cavity, with lightly improved hearing level.@*CONCLUSION@#Obliteration of a canal wall down huge mastoid cavity by Palva graft with autologous bone pate is a reliable and effective technique that results in a small, dry, low-maintenance mastoid cavity. The short-term efficacy of simultaneous III tympanoplasty is satisfactory if patient selection is suitable.

ABSTRACT

BACKGROUND: Amfetamine, one of the levarterenols, can improve the recovery of motor function of animals after cerebral ischemia. Chlorimipramine can inhibit the re-intake of 5-serotonin and levarterenol and improve the level of them in brain. OBJECTIVE: To observe the effect of chlorimipramine on motor function of rats with focal cerebral ischemic injury.DESIGN: Randomized grouping and controlled animal study. SETTING: Neuropsychiatry Team of Aerial Clinical Medical Department of the Fourth Military Medical University of Chinese PLA. MATERIALS: The experiment was completed in the Morphological Laboratory of Aerial Aerospace Medical Department of the Fourth Military Medical University of Chinese PLA. All the 24 SD rats were divided into sham operation group, ischemia group and ischemic medication group with 8 in each group. Rats in ischemic medication group were perfused with 10 mg/kg chlorimipramine solution (2.5 g/L) once a day through mouth 24 hours after ischemia, and rats in sham operation group and ischemia group were perfused with the same volume of distilled water through mouth. METHODS: Middle cerebral artery (MCA) models of ischemia/reperfusion rats were established with inserted lining method. After modeling, the operations were performed as followed: ① holding test of net screen: Net screen was put horizontally, and then rats were put on it. One side of the screen was raised gradually; then it was turned over 125° within 2 s, and maintained at this placement. Time of holding rats on net screen was recorded. ② Test of struggling to tear off rubberized cloth: 0.5 cm2 medical rubberized cloth was adhered to the ventral of anterior claws of rats, and then rats were sent to the observing box to record the time of tearing off rubberized cloth. 1, 3, 7, 14 and 28 days after operation were the observing time points. MAIN OUTCOME MEASURES: Times of holding rats on the net screen and tearing off the rubberized cloth of rats with focal cerebral is chemia.RESULTS: ① Time of muscular strength test of rats in ischemia group and ischemic medication group was shorter than that of rats in sham operation group, and there was significant difference [3 days after operation:(54±4), (20±5) and (21±4) s in sham operation group, ischemia group and ischemic medication group respectively, P < 0.01]. Holding time was longer in ischemic medication group than that in ischemia group, and there was significant difference [28 days after operation: (51±5) and (54±5) s in ischemia group and ischemic medication group respectively, P < 0.05]. ②Time of tearing off the rubberized cloth was longer in ischemia group and ischemic medication group than that in sham operation group, and there was significant difference [3 days after operation: (47±9), (188±20) and (172±22) s in sham operation group, ischemia group and ischemic medication group respectively, P < 0.01]. The time in ischemic medication group was shorter than that in ischemia group, and there was no significant difference (P > 0.05).CONCLUSION: Chlorimipramine has great effect on the recovery of muscular strength after focal cerebral ischemia, but has poor effect on sense and refined motor function, which is in coincidence with the poor recovery of refined motor function of limbs after hemiplegia.

ABSTRACT

To illustrate the location of HSP-70 (heat\|shock protein\|70)correlative response in acute stage between middle and inner ear when mammalian middle ear infected by bacteria, and investigate the possibility to cause autoimmune inner ear injure by HSP-70 that induced by otitis media. An animal model of acute otitis media was established by inoculating Klebsiella pneumoniae (K.p) into middle ear of guinea pigs. Animals were sacrificed 1,3,5 and 7days after inoculation.Investigation of the locations of HSP-70 in middle ear mucosa and cochlea structures were detected by immunocytochemical technique .The results showed that light stain of HSP-70 existed in the superficial epithelium cells of normal mucosa and cochlea structures such as stria vascularis, spiral ligament and Corti′s organ.Heavy stain appeared in the same sites in the infected ears . No different positive sites was observed in all animals.Our findings suggested that HSP-70 were expressed in middle ear mucosa and inner ear structures during acute middle ear infective stage.The HSP-70 provide a material base for causing autoimmune inner ear injure during middle ear bacteria infection.
